SOUTH BAY WOLVES SUMMER BASKETBALL CAMP — PARTICIPATION WAIVER, MEDICAL AUTHORIZATION & CODE OF CONDUCT
I, the undersigned parent or legal guardian of the participant listed above, give permission for my child to participate in the South Bay Wolves Summer Basketball Camp.
I understand that participation in basketball activities, including practices, drills, conditioning, scrimmages, and games, involves inherent risks including, but not limited to: sprains and strains, broken bones, concussions, cuts and bruises, heat-related illness, serious injury or, in rare circumstances, permanent disability or death. I acknowledge these risks and voluntarily allow my child to participate.
To the fullest extent permitted by law, I release and hold harmless South Bay Wolves Basketball, its directors, coaches, volunteers, employees, facility owners, and affiliates from any claims, demands, damages, or liability arising from participation in camp activities, except where caused by gross negligence or willful misconduct.
MEDICAL AUTHORIZATION
In the event I cannot be reached during an emergency, I authorize camp staff to obtain emergency medical treatment for my child. I understand I am financially responsible for any medical expenses incurred.
CAMP POLICIES
Camp fees are non-refundable unless otherwise approved by South Bay Wolves. Camp schedules may change because of weather, facility availability, or unforeseen circumstances. Campers are expected to arrive on time each day and wear proper athletic clothing and basketball shoes. Campers should bring a water bottle, lunch (Full-Day Camp), a positive attitude, and a willingness to learn. South Bay Wolves is not responsible for lost, stolen, or damaged personal belongings.
PLAYER CODE OF CONDUCT
Every camper agrees to: respect coaches, teammates, officials, and staff; demonstrate good sportsmanship; listen and follow coach instructions; encourage teammates; practice safely; use appropriate language at all times; respect equipment and facilities; and give maximum effort while maintaining a positive attitude.
Players will NOT: bully or intimidate others; fight or display aggressive behavior; use profanity or inappropriate language; damage property; leave the gym without permission; or possess tobacco, alcohol, vaping devices, drugs, or weapons. Failure to follow these expectations may result in disciplinary action, including removal from camp without refund.
PARENT/GUARDIAN CODE OF CONDUCT
Parents agree to: treat coaches, officials, players, and other families with respect; encourage effort rather than criticize mistakes; refrain from coaching from the sidelines; address concerns respectfully and privately with camp staff; and help create a positive environment for every participant. Unsportsmanlike or disruptive behavior by parents may result in removal from camp events.
By checking the boxes below, I acknowledge that I have read and understand this Participation Waiver and Code of Conduct, I understand the risks associated with participation, my child is physically able to participate, and I agree to abide by all camp rules and policies.
