Liability Waiver & Release

Required — please read the full waiver carefully before signing.

SOUTH BAY WOLVES YOUTH BASKETBALL — PARTICIPANT WAIVER, RELEASE OF LIABILITY, AND ASSUMPTION OF RISK

In consideration of my child's/ward's participation in South Bay Wolves Youth Basketball ("Program"), I, the undersigned parent or legal guardian, hereby agree to the following:

1. Assumption of Risk. I acknowledge that participation in basketball and related athletic activities involves inherent risks of injury, including but not limited to sprains, fractures, concussions, and other physical harm. I voluntarily assume all such risks on behalf of my child/ward.

2. Release of Liability. I hereby release, waive, discharge, and covenant not to sue South Bay Wolves, its coaches, volunteers, officers, directors, employees, and agents (collectively "Released Parties") from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child/ward while participating in the Program or while on premises used by the Program.

3. Medical Authorization. In the event of an emergency, I authorize the Released Parties to obtain emergency medical treatment for my child/ward if I cannot be reached. I agree to be responsible for all medical expenses incurred.

4. Photo & Media Release. I grant South Bay Wolves the right to photograph and/or video record my child/ward during Program activities and to use such images for promotional, educational, and social media purposes without compensation.

5. Code of Conduct. I agree that my child/ward and I will abide by the South Bay Wolves Code of Conduct. Violations may result in suspension or removal from the Program without refund.

6. Refund Policy. I understand that registration fees are non-refundable once the season begins. Requests for refunds prior to the season start must be submitted in writing to [email protected].

7. Severability. If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.

By checking the box below, I acknowledge that I have read, understand, and agree to the terms of this Waiver and Release on behalf of myself and my child/ward. I confirm that I am the parent or legal guardian of the registered player and am authorized to sign this agreement.
